Health Targets in Primary Health Care
(System Level Measures)

Health Targets are a set of system level measures specifically designed to improve the performance of health services that reflect significant public and government priorities. They provide a focus for action. The impact they make can be measured to see how they are improving health for all New Zealanders. 

Nelson Bays Primary Health (NBPH) is committed to meeting Government Health Targets and works closely with local health care providers to support them.

Increased Immunisation
95% of 8-months-olds will have their primary course of immunisation (6 weeks, 3 months and 5 months immunisation events) on time.

Better help for smokers to quit
90% of Primary Health Organisation (PHO) enrolled patients who smoke have been offered help to quit smoking by a health care practitioner in the last 15 months.

Before School check referrals
Referrals will be sent onwards to providers following these checks
