An exciting opportunity has become available in our Richmond based office for a Community Education Coordinator (Heart & Diabetes) for 20 hours per week to ensure the provision of community based self-management programmes across Nelson Bays.

The Community Education service provides high quality, evidence-based programmes designed to empower those at risk of, or with heart disease and type 2 diabetes, with self-management skills to promote health and well-being.

We require the successful applicant to be a practicing

health professional with relevant clinical expertise in self-management support, and experience in group adult education/facilitation.

Smokefree Facilitator - Primary Health Team

Nelson Bays Primary Health (NBPH) is the Primary Health Organisation for the Nelson Tasman region. PHOs lead and coordinate primary health care within the local community.

 An exciting opportunity has become available in our Richmond office for the above role for 25 hours per week, which supports general practice and the wider community health services, to offer and support smoke free conversations.

The role will also support the Primary Health team at Nelson Bays Primary Health with a variety of administration tasks.

We require the successful applicant to have behaviour change and motivational interviewing experience and it is desirable to have experience in smoking cessation, primary care and/or Medtech and being a WIZZ with databases and data entry is highly desirable.

We are looking for dynamic Registered Nurses to join our amazing team within the aged care and acute admissions area. We are looking to fill these roles with permanent FTE of 1.2 (96 hours per fortnight) and there is also fixed term FTE of 0.3 (24 hours per fortnight) until March 2020.
