Health Care Home (HCH) Facilitator 0.8 FTE (Marlborough/Nelson District Wide) Fixed Term 2 years

The ideal candidate would be a qualified health professional i.e. Registered Nurse, Social Worker or an Occupational Therapist etc, with recent experience working in primary care, and have change management experience and an understanding of LEAN Methodologies.  

 You will:

·         have advanced relationship management and communication skills

·         be process orientated

·         have experience in facilitation

·         have the ability to coach and mentor a practice team to navigate the HCH change process

·         be a team player

·         and live in either Marlborough or Nelson Bays districts and be able to travel across the two districts
