Locality Care Coordinator & Kaiawhina
Full time: 80 hours per fortnight
Fixed term: Parental Leave Cover
Are you a skilled, culturally competent healthcare professional with excellent organisation, communication, and relationship management skills?
This is a unique and exciting combined role, available for an anticipated six month period to cover parental leave of a permanent team member.
A Locality Care Coordinator contributes to the Locality Care Coordination Team which is based in Nelson. By strengthening coordinated care, people are much more likely to be identified and provided with the right level of support for their needs. Therefore, the purpose of the Locality Care Coordination Team is to help provide an embedded and coordinated approach to people’s health care delivery by connecting with Health New Zealand – Te Whatu Ora teams, NGO’s, community organisations, social services, and General Practices teams within the locality. This role is also an opportunity to enhance health equity by supporting effective delivery of health services to priority populations.
We require the successful applicant to:
· Have a minimum of 2 years post qualifying experience in health
· Be experienced in health services collaboration and coordination
· Experience working in a care coordination or an MDT or IDT model of healthcare delivery
· Have cultural competency to support our diverse community
· Experience using new information technology
· Can demonstrate sound problem-solving skills and is solution focused
· Positive attitude with a realistic outlook
· Energetic and motivated, demonstrating flair and initiative
· A good sense of humor
· Takes ownership of own decisions in order to meet responsibilities of the role
· Works from a strong values base
