Advance care planning is a process that assists you and your whānau to identify your personal preferences and consider treatment and care options that can be incorporated into plans for your future health care if you become unable to make decisions for yourself.

This service is for everyone. You may be perfectly healthy and would like to plan and prepare for the worst-case scenario, e.g., when a sudden unexpected illness happens. You may want to plan because you have a life-threatening illness or a long-term condition.

This service will help with:

  • The service runs advance care planning and serious illness conversation guide workshops for health professionals at Nelson Bays Primary Health and community workshops for the public at various locations across the Nelson Bays region, including aged residential care facilities and through Nelson Tasman Age Concern

  • Health professional training can also be done face-to-face at general practice and aged residential care facilities
